A new, free preventive program has been operating in Poland since May. Thanks to it, all people over the age of 20 can benefit from diagnostic tests, health advice and prevention. How to use it? We provide step -by -step instructions.

Step 1: Survey

To participate in the My Health program, you must first fill out surveyin which the patient will be asked, among others, about education and social conditions, weight, growth or the occurrence of civilization diseases in the family.

The survey can be completed via the patient's online account, the Mojeikp application or at the Primary Health Care Clinic (Item). If the patient needs help in completing the form, then a medical worker can support him in this clinic.

How to complete the survey via IKP? It is available in the “Prevention” tab. After going to it, select “My health program” and then enter the “My health survey” tab. If the patient uses the Mojeikp application, he should go to the “Prevention” tab and choose the “My health survey” option.

You should book a few to several minutes to complete the form. This is important because it should be filled at once. The survey cannot be saved and completed its supplementation, for example, in a few days.

People aged 20-49 fill the survey every five years, and those who are 50 years old-every three years. During the electronic completion of the form, the patient will see the data of his physician POZ or information that he has not chosen it. This is a key moment, because the survey will go to the institution where this medic is working, and if the choice has not been made, there will be no one to pick it up and the patient will not receive an examination.

Step 2: Research

Based on the answers given in the survey, they will be commissioned tests. A referral is issued by a medical worker in item He will also inform the patient about the possibility of testing. The facility has 30 days for such action. The basic scope of diagnostic tests includes:

  • Blood morphology,
  • glucose
  • creatinine (with EGFR-allows detecting irregularities in the functioning of the kidneys), lipidogram (total cholesterol, LDL, HDL, triglycerides-this is crucial in assessing the risk of cardiovascular disease
  • TSH (thyrotropic hormone – basic examination ordered in diagnostics of thyroid disease),
  • General urine test

Depending on the age and result of the survey, this range can be expanded with:

  • ALAT, ASPAT and GGTP (Research panel to detect abnormalities in the functioning of the liver)
  • Total dog (in men – diagnostics of prostate cancer),
  • anti-HCV (hepatitis C),
  • Lipoprotein (A) (as part of the program this test is performed once in life – it allows you to detect hereditary metabolic diseases such as family hypercholesterolemia early.

Step 3: Visit

After testing, the patient receives an invitation to summary visit. It takes place at the POZ clinic within 14 days of the moment the facility receives the results. After analyzing the results and examining parameters such as pressure, heart rate, weight and growth, a medical employee will prepare an individual health plan (IPZ). It may include medical recommendations, but also preventive or planned examinations and vaccination.

Who can use the My Health program?

The program is addressed to every person who is 20 years old. If the patient has used research as part of the already completed program prevention program, it is possible to participate in my health after a year after they are completed. The program is implemented by primary care facilities that have a contract for the implementation of services with the National Health Fund.
